Psychomotor assessment: report structure and template to download

The psychomotor assessment process (prescription, clinical history, standardised tests, feedback), the structure of the report, and the areas evaluated. Template to download.

Countries covered : France

Performed under medical prescription, the psychomotor assessment is a procedure that contributes to the medical diagnosis. It combines standardised tests and clinical observation, resulting in a written report sent to the doctor.

This page details the framework, the process, the areas evaluated, and the structure of the report, and offers a ready-to-use template to download.

The framework to know

The four-step process

  1. The interview (clinical history). Psychomotor, medical, family, and school history; reason for the assessment and any additional assessments already completed.
  2. The evaluation. Standardized tests (strict scoring, calibrated by age group) coupled with clinical observation, in free and structured situations.
  3. Analysis and writing. Scoring, correlating data, and writing, often taking several days to several weeks.
  4. Feedback. Oral feedback and delivery of a written report explaining the psychomotor profile, objectives, and follow-up.

Areas evaluated

The psychomotor therapist combines a quantitative evaluation (calibrated scores) and a qualitative reading (clinical observation) of:

  • Muscle tone and posture: tonic regulation, postural adjustments, endurance.
  • Gross motor skills: static and dynamic balance, coordination.
  • Fine motor skills: dexterity, grip, manipulation.
  • Graphomotor skills: writing, speed, legibility.
  • Praxis: ideational, ideomotor, constructive.
  • Sensory perception and integration: visuospatial, tactile, proprioceptive.
  • Attention and executive functions: inhibition, flexibility, planning.
  • Spatiotemporal orientation and psycho-affective and relational sphere.

Structure of a psychomotor assessment report

  1. Identification and prescription. Patient, prescriber, prescription, date, reason.
  2. Clinical history. Psychomotor, medical, family, and school history.
  3. Behaviour and clinical observations. Body ease, emotional reactions, strategies, cooperation, fatigability.
  4. Results by area. Calibrated scores and qualitative analysis, area by area.
  5. Synthesis, psychomotor profile. Weaknesses and resources, linked to the clinical history.
  6. Conclusion. Psychomotor diagnosis (to be validated by the doctor), response to the request.
  7. Plan and recommendations. Areas of intervention, referrals if needed.

Feedback and transmission

The oral feedback covers the entire report with the patient and/or their legal representatives: the profile is explained, connections are made, and objectives are discussed. The written report is then sent to the prescribing doctor as well as to the patient or their family.

Saving time on writing

Between in-session observations, scoring, and formatting results, writing reports is highly time-consuming. Start with a fixed template, standardise your synthesis and plan formulations, and dictate your observations right after the session.

This is exactly what Delta does: you capture your observations live or via post-session dictation, and the tool generates a structured report based on your template, with a longitudinal patient memory that reuses the patient history from one assessment to the next. HDS hosting, GDPR compliant.

Is the psychomotor assessment reimbursed?

Not by the Assurance Maladie in private practice. Certain supplementary health insurance companies or schemes (MDPH for children) may contribute to the funding.

Who can perform a psychomotor assessment?

Only a State-Certified psychomotor therapist. The assessment contributes to the medical diagnosis, which is then validated by a doctor.

What does the report contain?

Identification and prescription, clinical history, clinical observations, results by area, synthesis (psychomotor profile), conclusion, and therapeutic plan.

Which areas are evaluated?

Muscle tone and posture, gross and fine motor skills, graphomotor skills, praxis, sensory perception and integration, attention and executive functions, spatiotemporal orientation, and psycho-affective sphere.
