The structure of an initial dietetic assessment
The first consultation (often 60 to 90 minutes) forms the basis of all subsequent care:
- Identification and reason for consultation. Patient, date, referring clinician if applicable, reason for attending (weight loss or gain, a medical condition, an eating disorder, pregnancy follow-up…).
- History taking. Lifestyle, physical activity, health status, medical history, treatment, intolerances and allergies.
- Dietary intake assessment. A 24-hour recall, a weekly record or food diary, shopping and food preparation habits, meal patterns.
- Anthropometric data. Weight, height, BMI, waist circumference and, where appropriate, body composition.
- Dietetic diagnosis. Analysis of intake and any differences from nutritional needs and goals.
- Strategy and goals. Nutritional care goals agreed with the patient that are realistic and measurable.
- Plan and support. Dietary plan, suggested daily intake, example menus, nutrition education.
- Conclusion and handover. A signed and dated report; a letter to the treating physician where appropriate.

The follow-up consultation: what to record
This shorter consultation (30–45 minutes) reviews progress: weight and measurements, achievement of previous goals, adherence and barriers, adjustments to the plan and new goals. This is where a longitudinal record across appointments saves the most time.

Private-practice dietetic consultations are generally not covered by France's statutory health insurance (Assurance Maladie), but complementary health insurers sometimes reimburse part of the cost.
